However, machine failures often cause downtime, defective products, and increased maintenance costs. This study aims to analyze failure modes in injection molding machines using the Failure Mode and Effects Analysis (FMEA) method to improve production reliability at the South Sumatra Palm Oil Processing Industry. The research employed a quantitative descriptive-analytical approach, using observations, interviews, documentation, and maintenance records collected during August 2025. The analysis focused on critical machine components, including the mold, heater band, cutting system, hydraulic pump, and thermocouple. The results showed that the mold failure coded M1 had the highest Risk Priority Number (RPN) of 140, followed by the cutting failure C1 with 120 and the mold closing failure M2 with 108. Pareto analysis indicated that approximately 60% of operational disruptions were caused by four dominant failures. The proposed preventive maintenance and inspection strategies are expected to significantly reduce operational disruptions and improve production reliability.</p> Bayu Wahyudi, Asrul Fole, Muhamad Alfi Syahrin, Nurul Binti Gafar, Yasmin Copyright (c) 2026 Journal of Industrial System Engineering and Management https://creativecommons.org/licenses/by/4.0 https://journal-jisem.com/index.php/jisem/article/view/75 Tue, 30 Jun 2026 00:00:00 +0000 Halal Supply Chain Risk Analysis Using an Integrated FMEA–SCOR and Monte Carlo Simulation Approach to Assess Blockchain Technology Adoption Feasibility in Indonesian Halal Food MSMEs https://journal-jisem.com/index.php/jisem/article/view/73 <p>Indonesia's halal industry is one of the fastest-growing components of the national economy, with the food and beverage sub-sector posting 5.87% year-on-year growth in 2024 and total halal consumption reaching IDR 545.6 trillion in 2022, yet only 4.4% of the country's roughly 28 million Micro and Small Enterprises (MSEs) had been halal-certified by May 2024, making supply chain readiness a critical concern ahead of the mandatory halal certification deadline of October 2026. This study integrates Failure Mode and Effect Analysis (FMEA) with the Supply Chain Operations Reference (SCOR) model to identify and prioritize risk points along the MSME halal food supply chain, and then applies a 10,000-iteration Monte Carlo simulation to quantify the ex-ante reduction in aggregate Risk Priority Number (RPN) achievable through blockchain technology adoption. Twenty-eight risk points were mapped across the Plan, Source, Make, Deliver, and Return processes, with an aggregate Total RPN of 4,180 before mitigation; the five highest-RPN risks all relate to traceability gaps and halal-document integrity, with the Make stage contributing 39.2% of total risk. Simulation results show that blockchain adoption produces a non-linear, diminishing-returns reduction profile: 21.2% risk reduction at 25% penetration, 41.4% at 50%, 62.0% at 70%, and 79.3% at 90%. The optimal investment point is estimated at p ≈ 0.70, beyond which marginal benefits decline sharply. However, many SMEs still lack a structured performance measurement system that integrates economic, environmental, and social aspects. This study aimed to develop a sustainability-based Key Performance Indicator (KPI) framework to assess distribution performance and improve supply chains for soybean-processing SMEs in Batam City, Indonesia. A quantitative case study approach was employed, using the Triple Bottom Line (TBL) concept, the Delphi method, and the Analytic Hierarchy Process (AHP). The results identified and validated 18 KPIs, comprising 6 economic, 6 environmental, and 6 social indicators. AHP analysis revealed that the Economic dimension received the highest weight (0.52), followed by the Environmental (0.29) and Social (0.19) dimensions. At the indicator level, Order Fulfillment Rate (0.109) and Delivery Reliability (0.109) achieved the highest global weights, followed by On-Time Delivery (0.104) and Carbon Emission (0.087). The consistency test yielded a CR value of 0.062, indicating reliable expert judgments. The research employed a mixed-methods approach involving observations, interviews, Focus Group Discussions (FGD), and questionnaire surveys conducted on 157 respondents from agroindustrial SMEs producing banana chips in East Luwu, Indonesia. Data analysis was performed using Structural Equation Modeling–Partial Least Squares (SEM-PLS). The results revealed that RM positively affects SR with a path coefficient of 0.384, while CE demonstrated the strongest influence on SR with a coefficient of 0.451. Furthermore, SR significantly influenced SC with a coefficient value of 0.612. Mediation analysis confirmed that SR significantly mediates the relationships between RM and SC (0.235) as well as CE and SC (0.276). The factory continuously faces occupational accident issues in the form of blistered skin, itchy skin, eye irritation, and sprained feet, all of which disrupt the production process. This study identifies work accidents occurring at Tahu Bintang Salma Factory and proposes occupational health and safety (OHS) control methods to address these problems. Data were collected and processed using the Job Safety Analysis (JSA) method. Results revealed eleven potential hazards: spinal pain, bruised hands, bruised feet, sprained feet, tailbone injury, blistered skin, eye irritation, reddened skin, skin irritation (itching), bruised fingers, and hand lacerations. These risks were categorized into four levels: six low-risk, three moderate-risk, and two high-risk incidents.
